Community Care Services :

Dove Home Care provides programs to take care of the disabled and elderly. We offer a wide range of services from personal care such as Bathing, Grooming, Toileting, Laundry, Shopping etc and skilled services such as nursing and therapy.

Primary Home Care/Family Care (PHC/FC)

PHC is a non-technical, medically related personal care service prescribed by a physician as part of a client's plan of care. PHC is available to eligible adults and children whose health problems cause them to be functionally limited in performing activities of daily living.

Community Based Alternatives (CBA)

The Community Based Alternatives (CBA) program provides home and community-based services to persons age 21 and older who would qualify for nursing facility (NF) care. These services are provided as cost-effective alternatives to placement in nursing facilities.


The eligibility of the CBA, PHC/FC participant will be determined by the DADS case manager.
